Terms and Conditions
This Membership Agreement (“Agreement”) is between Skincare Lab (“Studio”) and the enrolling member (“Member”). A written copy of this Agreement will be provided at the Member’s first visit for review and signature.
1. Membership Options
Basic Membership — $160/month
6- or 12-month option
Includes:
• One Skincare Lab Facial each membership month
• 10% discount on all skincare products
• 25% off one additional treatment within the same membership month, including laser hair removal
Luxe Membership — $240/month
6- or 12-month option
Includes:
• One treatment of your choice each membership month, excluding laser hair removal and Nova Skin Laser Resurfacing
• 15% discount on all skincare products
• 50% off one additional treatment within the same membership month, including laser hair removal
Note: Laser hair removal and Nova Skin Laser Resurfacing are excluded from the included monthly Luxe treatment. Laser hair removal is eligible for the additional-treatment discount.
2. Term and Renewal
Memberships are offered with 6- or 12-month options. Members may cancel at any time, including during the selected term, by providing at least 30 days’ written notice. No early termination fee or penalty applies, and the Member is not required to pay for remaining months after cancellation takes effect. After the selected term, membership continues month to month unless canceled.
3. Payment Terms
Membership fees are automatically charged each month to the Member’s authorized payment method. Monthly billing continues whether or not the included treatment is booked or used. Additional products and services are charged at purchase or service. If a scheduled payment fails, membership benefits may be suspended until payment is completed.
4. Cancellation Policy
Membership may be canceled at any time by providing the Studio with at least 30 days’ written notice. There is no cancellation or early termination penalty. Regular billing and membership benefits continue during the notice period and stop when cancellation takes effect. Fees already paid are not automatically refunded upon cancellation.
5. Booking and Appointment Policy
Appointments must be scheduled in advance and are subject to availability. Please provide at least 24 hours’ notice whenever possible to reschedule. In exceptional circumstances, rescheduling requests are accepted up to 6 hours before the appointment. Requests made less than 6 hours before the appointment are subject to availability and are not guaranteed. If the Member does not attend an appointment and gives no notice (no-show), the included monthly treatment is considered used and is forfeited.
6. Membership Benefits
Each paid membership month includes one eligible treatment. The included treatment must be used within that membership month; unused treatments expire and do not roll over. Membership benefits are personal, non-transferable, and non-refundable. The additional-treatment discount applies to one eligible additional treatment during the same membership month and cannot be combined with other promotions or offers.
7. Health and Safety
Members must complete the required health and safety questionnaire before treatment and inform the Studio of relevant medical conditions, medications, allergies, pregnancy, and any changes that could affect treatment safety. The Studio may recommend an alternative treatment, postpone an appointment, or decline a treatment when medically or professionally appropriate.
8. Liability
Treatment responses and results vary by individual. Members agree to provide accurate health information and follow applicable pre- and post-treatment instructions. The Studio will use appropriate professional care and safety practices. Nothing in this Agreement is intended to waive rights or limit liability where doing so is prohibited by applicable law.
9. Governing Law
This Agreement is governed by the laws of the State of Florida. Any dispute will be handled in accordance with applicable Florida law.
10. Agreement
By signing the written Agreement provided at the first visit, the Member acknowledges reviewing and accepting the membership benefits, recurring monthly payment terms, appointment and no-show policies, cancellation requirements, and expiration of unused monthly treatments. Payment authorization is handled separately through the Studio’s existing authorization process.
